Mazdutide is an investigational dual GLP-1/glucagon receptor agonist developed for obesity and type 2 diabetes, primarily in trials outside the US. It is not FDA approved.
Dosing Reference
Reconstitution
Add 2 mL bacteriostatic water to the 10 mg vial. Resulting concentration: 5 mg/mL.
| Dose | Syringe Units | mL Volume | Schedule |
|---|---|---|---|
| 3 mg | 60 units | 0.6 mL | SubQ, once weeklyPhase 3 starting dose |
| 4.5 mg | 90 units | 0.9 mL | SubQ, once weeklyPhase 3 titration dose |
| 6 mg | 120 units | 1.2 mL | SubQ, once weeklyPhase 3 mid dose |

Math assumes U-100 insulin syringes (1 mL = 100 units). Verify your syringe matches before use. Round half-units to the nearest visible mark.
Reconstitution
Mazdutide (IBI362 / LY3305677) is an investigational GLP-1/glucagon dual receptor agonist (Innovent / Eli Lilly), approved in China for weight management. It is supplied as a pre-filled pen or lyophilized vial for subcutaneous use. Compounded research-peptide forms are not FDA-approved.
For a compounded 10 mg vial reconstituted with 2 mL, the concentration is 5 mg/mL; a 6 mg dose draws to 120 units (1.2 mL) on a U-100 syringe. Store per manufacturer directions; compounded forms are not FDA-approved.
Cycling Protocol
Phase 3 / approved-in-China protocols titrate mazdutide weekly: 3 mg → 4.5 mg → 6 mg → up to 9 mg once weekly over several weeks, then maintained long-term. No fixed cycle end; used as chronic weekly therapy.
Routes of Administration
Subcutaneous injection (only established route): Once weekly into the abdomen, thigh, or upper arm. Any time of day, independent of meals.
No oral, intranasal, or intravenous route is established. Compounded forms are not FDA-approved.
Stacking Protocols
| Stack | Components | Purpose | | --- | --- | --- | | Dual agonist + Lifestyle | Mazdutide + diet/exercise | Approved-in-China weight protocol |
Combining multiple GLP-1 agonists is not recommended. Mazdutide is approved in China but not FDA-approved in the US.
Side Effects & Safety
- Nausea, vomiting, diarrhea, constipation — common, usually transient
- Hypoglycemia — risk, especially combined with insulin/sulfonylureas
- Pancreatitis — rare; discontinue if suspected
- Injection-site reactions — common
- Thyroid C-cell tumors — in rodents; avoid in MTC/MEN2 history
Mazdutide is approved in China but not FDA-approved in the US. Compounded research-peptide forms are not FDA-approved.
Research Basis
Mazdutide is a GLP-1/glucagon dual receptor agonist (oxyntomodulin analog), developed by Innovent Biologics / Eli Lilly.
Mechanism: Dual GLP-1 and glucagon receptor agonism — GLP-1 effects (insulin secretion, appetite suppression) plus glucagon effects (increased energy expenditure, hepatic fat oxidation), supporting both weight loss and metabolic benefit.
Clinical evidence: Approved in China for chronic weight management (2024) based on phase 3 trials demonstrating significant weight loss. Not FDA-approved in the US.
Approved in China; not FDA-approved in the US. Compounded research-peptide forms are not FDA-approved.
