Insulin

Insulin is an essential peptide hormone that regulates blood glucose and has been used as a medicine since 1923. Multiple FDA-approved formulations exist for diabetes management.

Reconstitution

Add 10 mL bacteriostatic water to the 100 mg vial. Resulting concentration: 10 mg/mL.

Math assumes U-100 insulin syringes (1 mL = 100 units). Verify your syringe matches before use. Round half-units to the nearest visible mark.

Reconstitution

Insulin is FDA-approved for diabetes mellitus and is supplied as a solution/suspension in vials (U-100 = 100 units/mL) or pre-filled pens. It is not user-reconstituted from lyophilized powder except for specific concentrated formulations per manufacturer instructions. Compounded forms are not FDA-approved.

Standard insulin is U-100 (100 units/mL); a 10-unit dose draws to 10 units (0.1 mL) on a U-100 syringe. Concentrated U-500 (500 units/mL) is used for severe insulin resistance. Store per manufacturer directions; compounded forms are not FDA-approved.

Cycling Protocol

FDA-approved insulin dosing is fully individualized to carbohydrate intake, blood glucose, activity, and insulin sensitivity — there is no fixed cycle. Basal-bolus regimens use a long-acting basal once daily plus rapid-acting boluses before meals, titrated to glucose targets.

Routes of Administration

Subcutaneous injection (primary FDA-approved route): Into the abdomen, thigh, or upper arm; rotate sites. Rapid-acting (before meals), short-acting, intermediate, and long-acting (basal) formulations.

Inhaled (FDA-approved): Afrezza inhaled insulin before meals.

Compounded forms are not FDA-approved.

Research Basis

Insulin is the naturally occurring peptide hormone produced by pancreatic beta cells, FDA-approved for diabetes mellitus (multiple formulations).

Mechanism: Binds the insulin receptor to promote cellular glucose uptake, glycogen synthesis, lipogenesis, and protein synthesis; suppresses hepatic glucose production.

Clinical evidence: FDA-approved for type 1 and type 2 diabetes mellitus (1922 onward). Multiple formulations (rapid-, short-, intermediate-, long-acting, inhaled) with extensive clinical experience.
